They will accept your MD 16 hour training provided it is less than two years old. They did mine when I got it almost two years ago now.
Those applying for the first time will take 2 months or more.

No, I called and was told I had to sign a form that says of the information in my original application has changed. You can do it in person, or call and they will mail you the form.
