I found something very similar out at the BLM spot where I shoot. Looks like it was intentionally done to neutralize misfires. I'd never seen this before.
That little smiley in factory-new ammo happens in the case forming process when the die presses the sheet brass into a cup shape. A sloppy mandrel fit tears out the brass. I blame old, tired machinery.
