It's a design problem; the zinc alloy they are using is simply not strong enough for the use it's being put to. Some of the other competing guns have aluminum alloy slides (SR22, M&P-22), which should be able to take the stress better.
I was willing to give the benefit of the doubt to the P22 when it came out, but it seems like there's enough evidence of its weakness that it's reasonable to call it inherently flawed.
