When I filled mine out a few months ago I made a few mistakes. One was not signing the authorization for them to charge my credit card, and the other was not listing the rank or title of the local chief LEO. They returned the form to me with these flagged and I fixed it and sent it back. No problems other than taking a little longer for processing.
