I've seen PLENTY of mil-surps that will slam fire, and one Norinco SKS that would mag dump after racking the bolt. All it takes is a little cosmo, a burr or rough machining to get that firing pin to jam forward, and fire, possibly out of battery. Granted, haven't seen it on new commercial guns, but got into the habit of checking the breech face and pin, cleaning it well, and function checking before it's first range trip, then at the range loading only 2 rounds at a time till it seems to run fine. Can't always run to the range to chamber a round, can't always keep the muzzle from sweeping flesh especially when carrying, so it's really important to know your gun works properly, and load in a relatively safe direction.
