Md Designated Collector Form

I took the form and updated it to be an actual fill in the blanks form which can be opened with Adobe Reader, filled in, and then printed out. I figured it saves a lot of writing. The best thing to do is download, fill in and save as a different name.

Nice. I like filling them out on the computer when that feature is available

someone did this for the MD LTCH application when everyone thought they were going to get their rights back.
