Type in the name of your instructor from your score sheet, where it says search qualified instructor

Then, an upload box will appear

Double check your selections and see if maybe you checked something that you didn't mean too. If I remember correctly, somewhere it asked what my reason was and I put the spiel about the SC ruling.

In the field asking to Search for Qualified Handgun Instructor, start entering your instructor's last name until his/her full name appears, then accept it. You should then be able to upload the document. Leave the bottom part ("official reason") alone and you should be GTG. I THINK that's what I did.
