Form 922(g)(5)(B) Certification of Compliance - For a trust

Hearing rumors of trusts kicked back lacking this form. I read here:

The form has a First, Last and Middle Name box as well as one for Name of Corporation, Partnership, or Association (if applicable). Should I just put my trust name under the corporation/partnership/association box, then fill it out like guntrustlawyer.com says? My dealer recommended I submit this just incase.

You may have heard rumors of this, but I had a form 1 approved last month with no such certification.

I've sent it in with all of my form1s/4s. Takes 10 seconds to fill out, why take the chance?

Did you put the trust name under last name, or to the far right under corporation, etc?
