C&R form question

the final box for signature (#15) says sign here, and beside it to the right has a box entitled "Title".

What do I put for Title? My name? "Collector"? "Personal Use"?

also box 9, where it asks for my name and ssid, what does it mean by "position"?

I went to the one that's stickied for this thread but did not see it in the initial write up

I just put in collector. It was not on my old license before I renewed. This is something they added.

I put "licensee".
