    Is it required? I never put my SSN on NICS or state forms with no problems. Has anyone not put it in the form and been denied?

    I just missed it. (It's a stupidly laid out form in some places.)

    They sent it back with a letter requesting the SS#, and stating that if they didn't receive it back in 30(?) days, my check would be returned.

    I made the correction, sent it back, followed up with a phone call a month after that, and had it in three days after that.
