The more units in service the higher the probability of anything. (Glocks have shot more...than Hi Points. That's because no PD uses Hi Points.)
The reflex grasp is the Enoka response (Dr. Enoka). As was pointed out, keep your finger off the trigger & out of the trigger guard.

If you like the fact that a judge throws out your "BS" traffic ticket, then you probably love it when the judge is lenient on those who have their 4th DUI. OR driving while suspended for...
I have that same pistol in 40 and have shot nearly 7,000 rounds without experiencing one key hole. Over 6,000 rounds were Berry's plated flat points that I've reloaded.
Interested to learn what is / was causing this. I tend to agree that it's poor ammo.

Congrats!
Before going with a .223 / 556, check the restrictions on muzzle energy. Most cannot achieve the minimum.
I have two 6.8 SPC's and highly recommend that caliber.

I have a 550 & have my calibers pre-set on tool heads. There is great satisfaction to switching calibers in less than 10 minutes. Consistency is key, and this helps greatly.

If you don't already listen to the ProArms podcast, you should. The 87th podcast was about Spencer Newcomer of PA. He told his story of being in a shooting. Unbelievable! MAG-40 will teach you a lot of things to keep you out of those types of situations.

I'd use the shortest barreled 12 GA shotgun you have. It'll be easier to swing slowly. My SBE II is 24". I've used my son's youth model 1187 20 GA because it is light and very short. (He wanted to lug around my SBE II.)...
My $0.02...
20 GA will work fine. Killed a few with it as I was bored with my SBE II 12 GA.
Pattern your shotgun(s) in 5 yard increments with different brands & shot sizes from 5-40 yards. Your gun will prefer one over the others. Also, you'll see how easy it is to miss at those distances...
