Replacement MD W&C card that never arrived by mail

One of my coworkers got his approval notice email 8 weeks ago but never received his card in the mail. When he went to the website to request a replacement, he got a message that says he must send a notarized statement telling why he wants a replacement card. Has anyone else had this happen? Did he go to the right place and click the right link?

Some dumb A$$ named ToolAA lost his wallet today. I was at the gym and I think it fell out of my pants or gym bag while getting changed in the locker room. I guess expecting folks to have the common decency to turn it into lost and found is probably not reasonable. So much for the Howard...

Yes. I got my replacement in less than 10 days. I left my wallet at a restaurant the day before Thanksgiving. I also had to submit a notarized statement and pay $20 for the replacement. It was approved almost immediately. Sounds like you are on the right path
