How to file for a SBR?

I've read through many threads about SBRs but I still need some guidance. I would like to register an AR-15 lower as a SBR before October 1st. I hope this doesn't require a trip to the Pikesville barracks since I like on yhe lower eastern shore. What do I need to do?

Form 1 is good today and good after Oct 1, no change there.

Form 4 is good today but no good after Oct 1. If you have your PO or receipt for the form 4, then you are grandfathered per the legislation.
