I had a similar problem with the op rod unseating. The rab and receiver looked good so on a whim I replaced the op rod spring and it stopped happening. I wouldn't have guessed it'd make that big of a difference but it did. Maybe just lucky. Might be good to replace it anyway.

If it was dismounting when firing a new spring would not have fixed the problem. Only way it could dismount is out of spec tab or receiver. If it was dismounting when you charged te rifle it is more than likely shooter error and a new stiffer spring just changed the way you pulled back on the op rod

Op Rod tab specs, check with a Micrometer

    Piston Thickness: .190 to .155
Tab/Lug Height: .320 to .317 (under .310 calls for rejection)
    Tab/Lug Thickness: .100 to .095
    Tab/Lug Width: .265 to .235

    .156 thickness
    .305
    .083
    .246

The width does have a bit of a taper to it. I think its cooked.

Well it was dismounting when firing and occasionally when charging. I wasnt charging that rifle any different than I had been my other three for the past several years of almost weekly shooting. I replaced the spring and no problems since. Believe me I was in your camp thinking it wouldn't do anything but it did.
Like I said in my original pos, maybe I just got lucky.
