If one is applying for a new license, after having one years ago, would putting 'Don't recall' for the old FFL # be sufficient for Part B, questions 20, and 21?

Sent in my renewal and sent a courtesy copy to Sheriff DeWess in Carroll County along with my thanks for the good work done by him and his deputies... got a very nice personal response from the Sheriff. Pleasant surprise.
Ron Bateman used to send them back with a note to send it to the State Police. Guess why he's no longer Sheriff?

Anyone have a link to get a C&R application and fingerprint cards?

You don't need fingerprints for a C&R license.
