    NOTE 3-7-2012: as of this time, if you can afford it, MSI suggests submitting an application. DO NOT use your rent money or try this if you are having trouble paying your bills. Your money may be "held" for a while, worst case scenario, you could lose the amount and not get a permit..worst case.

    The Application Form contains instructions. It also has places to fill in for Private Detectives, Security Guards, etc.
    We will NOT cover anything outside of an Individual applying for a permit.
    If you are a Security Guard, Private Detective, or employee of a frim requiring weapons carry, please refer to your employer/agency/business for instructions.

    (need verification this is 100% correct) > The normal citizen with a clean record will fill out pages 7, 12, 13. If you do not have a clean background/record you MAY have to fill out Page 10, based on your answer from page 7. Convicted felons need not apply. If you think there is a reason you MAY not be approved, you should check with someone like a lawyer or an authority who can give you some guidance. Falsifying the application in any way is a Crime. They are going to perform a background check, they will find out anything you may be withholding, 'nuff said.

    Q: do you have to fill in employment record if applying as an individual? Or is that only for employees of firms requiring it for work?

    FINGER PRINTING

    2012 Finger Printing Rules from the Licensing Division can be found here:

    As of March 16, 2012 they will no longer accept inked finger print cards.
    This means you will need to go to a location that uses/supports Live Scan/CJIS. This technology takes a digital image of your fingerprints and directly uploads them into the database.
    You will be given a special receipt when you use Live Scan, this receipt should also be attached in your application packet when you submit it, make a copy of the receipt for your records, send the ORIGINAL in with your Application.

    PHOTOS
    Each applications requires 2 photos of yourself be attached. They should be 2" x 2" against a white background, showing head & shoulders.
    They must be taken within 30 days of the date you place on the Application. You may use computer generated digital photos, or go to a place CVS or Walgreens, etc. to get them taken.

    The App Form says 1 1/2" x 1 1/2" , however, there is an application form listed above, now, that states 2" x 2" (see above).
    *HoCo SHooter called to verify that 2"x2" size is fine, per MSP. They actually would like to change the form to read 2"x2".
    2"x2" is the norm for Passport and NFA/BATFE forms. It is what CVS, Walgreens and the rest are accustomed to. Thanks HoCo!

    Below is a portion of the printed information (cover sheet) to MSP CCW application. This may answer a number of question that have been asked. Here's what it says from their website:

    CHECK LIST – FOLLOW ALONG AND YOU WON’T GO WRONG

    1. Licensing Division – General Application – Completely, accurately, legibly, and truthfully completed in ink or by typing. MUST BE NOTARIZED

    2. Fingerprint cards (FBI and State) or receipt from an approved electronic fingerprint provider attached to the Licensing Division General Application.

    3 Two 2 X 2 inch passport type photographs of the applicant from the shoulders up. The photos should be in color, and the applicant should not be wearing a hat or sunglasses.

    5. Authorization for Release of Information form. MUST BE NOTARIZED

    6. Reachable references willing to speak on the applicant’s behalf. Difficulty locating references will further delay the application process.

    7 Details in response to a “yes” response of the application. This information must be complete and accurate as a background investigation will be completed. Use additional sheets, if necessary.

    8. Any documentation to respond to issues concerning drug or alcohol abuse and/or addiction from the health care provider as to treatment and prognosis.

    9 . Any documentation to respond to occurrences by the applicant where he/she was institutionalized (voluntarily/involuntarily) for any psychological evaluation or treatment to include documentation from the health care provider as to treatment and prognosis.

    10. If not a United States Citizen, provide one of the following documents; Permanent Resident Card, Employment Authorization Card, Birth Certificate (born abroad) and form I-94. If you are a Naturalized Citizen; provide one of the following; copy of your Naturalization Papers; a valid U.S. Passport.

    I went to the maryland livescan.. for $76

    1) printing
    2) background check fee
    3) pictures
    4) notary

    one stop shop in glen burnie.. and a total time in and out of ~10-15 min. you can NOT Beat that at the ~$20 premium over running around and doing it separately.

    you can refer people to me with questions too.. my out of state app has been mailed =)

    The MSP barracks in Leonardtown will not give out fingerprint cards to take over to the Sheriff to be fingerprinted.

    Saint Marys county Sheriff wants you to bring the cards with you.


    NOTE: anyone calling 410-764-4501 or 1-888-795-0011 (toll free) to make an appointment.

    The MVA will only schedule live scan for the next week only, anything beyond that - you will have to call at a later date.
