Five months sounds like a long time. They claimed about 1000 apps in process in early January, and mid October resident apps took about 14 weeks.

Yes.
The 14th Street address is the Reeves Bldg. MPD Security Officer Management Branch is there. They are responsible for DC certified CCW instructor licensing and renewals. The OP didn't mention that perhaps he was getting instructor certification, not a CCW.
But yes all regular CCW permits are issued at 300 Indiana Ave. NW.
