Found a 9mm like that once figured it was fired from a .40 S&W. Didn't have the cracks but it definitely stretched to fit the chamber.
It could have been held by the extractor.I"m surprised the round did not just push through the barrel. I guess enough inertial mass to overcome that the firing pin was able to make the primer go off.

I saw a qualification where the person fired one shot and the firearm wouldn't recycle. He taps racks and accesses, fires again. and the firearm wouldn't recycle. He asked me to look at his firearm and he had loaded 9 in a 40. Cases were steetched but the rounds were somewhat accurate and didn't appear to hurt the barrel. He borrowed a box of 40 and qualified despite the rest of his co-workers giving him the business.
Two ways to have firearm accidents - ignorance and carelessness. This was an experienced shooter. He had put 9mm back in a 40 caliber box after a previous range visit.
Ever seen someone drop a 20ga into a 12ga gun? It goes far enough forward to allow a 12ga behind it. You don't want to pull the trigger on that scenario. Two shells detonate with a total barrel obstruction from the 20ga shell = KABOOM!!!

Probably was an accident, that fortunately didn't result in an injury. On the other hand, what is really scary as that the shooter might have thought it was no big deal since it was small enough to still fit. I have seen and heard some pretty scary stuff while trying to pick up a deal or two at the local WalMart ammo counter.
After finding my 40S&W stash, I got a 1911 out of the safe and found that the 40 cartridge falls right through the barrel. I don't see how this was fired through a 45acp handgun. It could have, but I don't see how.
