I had the same issue on mine. It's that the throat of the barrel is too tight to feed the nose of the bullet in properly. That's why you get the ftf because the bullet went nose up on battery hitting the barrel throat like a wall vs a funnel, and the fte because the case was stuck in the tight...
If anyone finds themselves in a defensive situation where they fear harm and choose to pull their weapon, make sure you know what you're pointing it at, be aware of your surroundings, have a firm purchase on your gun and plant your feet if possible. Exactly the opposite of the woman in the photo.
I did get the issue resolved. As Photoracer said, I had the throat of the barrel reamed to smooth out the feed ramp to more of a cone shape. I sent the barrels to Rudy at Macon Armory. It turned out so well that I had my AR9 barrels done as well even though I didn't have the same issue.
