MD regulated firearm purchase application

I'am having trouble finding this application anywhere; the one we fill out at the gun shop when we purchase a firearm. I need to see the various questions that are asked. Does anyone have a link to it?

So there's not a pic of these forms somewhere? You need to stop at a barracks (or I guess a gun shop?)? I'm not looking for an actual app to fill out, just what it asks.

Isn't it almost exactly the same as the designated collector application?

Thus, the only inquiry that this Court should conduct is to determine whether the firearms prohibited by the Act are protected by the Second Amendment. Because they are, the Act is simply unconstitutional.

It asks what training you had (e.g. web course offered by MD--bring your card [multiple things can suffice])

Confirm not straw man purchase.

List of questions about criminal history, mental issues, pending issues

State of residence (again)

citizenship

The 77R is nothing like the Designated Collector's application.
