    What does the Instructor need to do, sign, or give me to complete my application?
    I looked at all 10 pages and I do not see anything for the Instructor to sign.

    How does or would the MDSP know you took the 8 hour training?

    Do I also need to put in reference's business information...etc?

    Do you still need to get the application notarized?

    I know I need to get my fingerprints done and two photos.

    Thanks for the help.

    If you're doing a renewal you will do the same as you did for your initial application plus include the course completion certificate that the instructor gives you.
