Duke's Defense up in Jessup (HoCo) is a great instructor and the course is all inclusive.
He also provides forms for Virginia and Florida non-resident permits if you would like them, and provides instructions on how to file them (VA permit just needs law enforcement to do the fingerprint card).

Wife and I did the course with him. Good guy, good course. Got all the paperwork for UT, VA, and FL, and all sailed through the process.

Mo at MSAR does a fantastic job and the class is dirt cheap. He also does fingerprints at no extra charge.

+1 for Mo at MSAR. Company is ASTC I believe an IP here too. Good price, good info, Mo is pretty funny too. I enjoyed his wise cracks he mixed in. Really helps because it is close to 4 hours by time you get all the required teaching and fingerprints done.
